ENTRY FORMS

To REGISTER for any TORHS America Regional and/or NATIONAL tournament, please go to the "TOURNAMENTS" tab and click on the logo for the event you would like to take part in.

Below you will find all forms you need for proper registration so that your process will be easier at all events.  At all events, your team will need to fill out one ROSTER FORM with all names and numbers filled out accordingly.  Each player will need to have RHA INSURANCE. The insurance form may be printed out by clicking below to download the form or by going to www.rollerhockeyalliance.com to pay by credit card.  This can only be done for the annual membership.  Lastly, all players will need to fill out the TORHS America WAIVER FORM.  After all of this is done, your registration will be complete.
